HCA Health: Benefits After Termination

Your 3-4 ID is about to stop working, and once it does, getting your benefits information from HCA Healthcare becomes a phone marathon. Here is what to do before and after that happens.

HCA Healthcare is the only healthcare company among the 22 employers we cover, and its systems reflect a hospital environment rather than a retail one. The 3-4 ID (a format like ABC1234) is your key to everything at HCA. It controls access to HCAhrAnswers (hcahranswers.com), payroll through Parallon, scheduling through My Scheduler, and benefits through HCARewards. When that ID gets deactivated after separation, you lose access to all of those systems at once.

The goal is to get what you need before that happens. If you are already locked out, this guide tells you which phone numbers actually work.

Step 1: While you still have access, save everything

Before your last shift, log into HCAhrAnswers (also known as the OneHR Portal or Identity Federation Portal, reachable at hcahranswers.com or hcaghr.com) and save or screenshot the following:

  • Your current health plan details (plan name, group number, covered dependents)
  • Your PTO bank balance
  • Your most recent pay stubs
  • Your 401(k) balance and vesting percentage
  • Your benefits enrollment confirmation
  • Any contact numbers for your facility’s HR

HCA does not have a mobile app for its 280,000 employees. Everything runs through the web portal, and shared workstations at hospitals use SSO or “Tap and Go” authentication. Once you are no longer badged into a facility, web access with your 3-4 ID is all you have, and that window is short.

Step 2: Understand when health coverage ends

HCA health, dental, and vision coverage ends on the last day of the month you separate. This applies whether you were a nurse, technician, administrator, or any other role.

COBRA is available for up to 18 months at full premium plus 2%. Given that healthcare employers tend to offer better-than-average health plans, COBRA premiums at HCA may be higher than what a retail worker would face, potentially $500 to $900 or more per month for individual coverage.

You have 60 days to elect COBRA or enroll in a Marketplace plan. Many former HCA employees, especially those coming from hourly or per-diem roles, find that Marketplace subsidies make those plans more affordable. Check Medicaid eligibility if your income drops low enough.

Healthcare workers transitioning between hospital systems may also be able to negotiate earlier benefits start dates with a new employer. This is more common in nursing and clinical roles where demand is high.

Step 3: Handle your 401(k) and retirement accounts

HCA’s 401(k) plan stays in place after you leave. Check your vesting schedule to confirm how much of HCA’s matching contributions belong to you. Standard options apply: leave it, roll to an IRA, transfer to a new employer, or cash out (10% penalty plus taxes if under 59 1/2).

Step 4: Determine your PTO bank payout

HCA uses a PTO bank system that varies by facility and department. Healthcare scheduling (12-hour shifts, rotating schedules, holiday rotations) creates PTO situations that are more complex than retail.

Whether your PTO bank gets paid out at separation depends on your facility’s policy and your state’s law. HCA operates 180+ hospitals across multiple states, and each facility may handle PTO payouts slightly differently.

Step 5: Get your W-2 after the 3-4 ID is deactivated

This is the number-one complaint from former HCA employees. Once your 3-4 ID is deactivated, you lose access to HCAhrAnswers, and Parallon (the payroll processor) access goes with it. The phone lines can be difficult to get through, especially during tax season.

Here is your plan:

First try: Log into HCAhrAnswers (hcahranswers.com) using your 3-4 ID. Navigate to My Pay for W-2 access. If it works, save a copy immediately.

If locked out: Call (844) 472-6797. Be patient. This line handles all former employee HR inquiries and gets backed up in January and February. Call early in the morning for shorter wait times.

Paper W-2: Mailed to your last address on file by January 31. If you moved after leaving HCA, updating your address requires calling that same (844) number, which is why people get stuck.

Employment verification: If a new employer needs to verify your HCA employment, use Thomas & Company with employer code HCA747.

Other benefits that end

HCARewards (the portal at hcarewards.lifeatworkportal.com with discounts on Amazon, Target, and other retailers) deactivates after separation.

Tuition reimbursement stops on your last day. If you are mid-program through an HCA-sponsored clinical education or nurse residency, contact your program coordinator about completion options.

Dependent scholarships through HCA are tied to active employment. Confirm with HR whether any scholarship awards in progress are affected by your separation.

EAP (Employee Assistance Program) access may continue for a short period after separation at some facilities. Check with your facility’s HR.
