Former Lowe’s associates get their W-2 at mytaxform.com using employer code 11116. That’s the fastest route, and it works even if your MyLowesLife access is gone. Work through the checklist below. If every step lines up, you’ll have a PDF of your W-2 in under 15 minutes.
Before you start: collect these items
- Your Sales Number (7-digit ID, on every paystub)
- Your Social Security Number
- Your date of birth
- The email address or phone number you used with Lowe’s
- Lowe’s employer code: 11116
- A recent paystub if you have one (useful for cross-checking)
If you can’t find your Sales Number, check any old Lowe’s paystub, the onboarding email from when you were hired, or call the Lowe’s HR line at the number on the corporate website. They can look it up with your SSN.
Primary path: MyLowesLife, if your access still works
- Go to myloweslife.com
- Enter your Sales Number and password
- Select Former Lowes Associate if prompted
- Navigate to My Wealth > Payroll Services > W-2 Services
- Download the W-2 for the year you need
This works for roughly 60-90 days after separation for most associates. After that, your MyLowesLife credentials stop working and you have to use mytaxform.com instead.
Backup path: mytaxform.com (works long-term)
- Go to mytaxform.com
- Enter employer code 11116
- Confirm Lowe’s logo appears (phishing check)
- Click Register Now for first-time users, or sign in if you registered previously
- Enter your Sales Number and SSN for verification
- Create a username, password, and PIN
- Go to Tax Forms and download your W-2
If the registration rejects your Sales Number or SSN, try again carefully. Three failed attempts locks you out for 24 hours.
Paper W-2: what to expect and what to do if it’s missing
- Lowe’s mails paper W-2s to your last address on file by January 31
- If you moved after leaving, check whether your address on file is current
- If the W-2 didn’t arrive by February 14, request a reissue
- Call Lowe’s HR at 1-844-HRLOWES (1-844-475-6937)
- Verify the mailing address on file
- Request an address update
- Request a W-2 reissue
Reissues take 10-14 business days. If April 15 is close, skip the mailed reissue and download from mytaxform.com.
If your identity verification fails
This is the most common problem with mytaxform.com. Causes and fixes:
- Name change: Use the name that appeared on your last Lowe’s paystub, not your current legal name (if you’ve changed it since). Lowe’s may not have updated.
- Sales Number wrong: 7 digits, no spaces, no dashes. Confirm you’re not using your SSN by mistake.
- SSN format: 9 digits, no punctuation
- Date of birth: Match exactly what Lowe’s has on file, not necessarily your actual DOB if Lowe’s recorded it incorrectly
- If two attempts have already failed, stop before the third locks you out
Call mytaxform.com support through the number on the login page, or call Lowe’s HR to request an email delivery of the W-2 as a password-protected PDF.
Double-check these numbers against your final paystub
Once you have the W-2, run through this quick comparison before you file:
- Box 1 (federal wages) matches YTD gross on final paystub minus pre-tax deductions
- Box 2 (federal income tax withheld) matches YTD federal withholding
- Box 12 codes for 401(k) (code D), HSA (code W), ESPP if applicable
- Box 14 for any Lowe’s-specific items (charitable giving, etc.)
- State boxes (15-20) for your work state(s)
- Name, SSN, and address on the form are correct
Mismatches go to a W-2c (corrected W-2) request through HR before you file. Don’t file with wrong numbers.
If nothing is working
- IRS Wage and Income Transcript at IRS.gov, available late May for prior tax year
- IRS Form 4852 (substitute W-2), filed with your return using last paystub figures
- File an extension (Form 4868) to push the deadline to October 15 while you sort it out
Form 4852 should be a real last resort. Processing slows, and IRS verification letters are common.
Tax documents that aren’t the W-2
If you participated in Lowe’s programs, you may have additional forms to track down:
- 1099-DIV for ESPP dividends (issued by the brokerage, not Lowe’s)
- 1099-B for any ESPP stock you sold during the year (also from the brokerage)
- 1099-R for any 401(k) distribution or rollover (from the plan administrator, typically Wells Fargo for Lowe’s)
- 1095-C for health insurance coverage (usually in mytaxform.com or Paperless Employee)
Each of these goes somewhere different than your W-2. Log into the brokerage and plan administrator accounts separately.
